Every plumbing & drainage form Queensland licensed plumbers and gas fitters need — Form 1, Form 4 (notifiable work), Form 7 (responsible person), Form 9 (backflow), plus per-council lodgement portals and trade-waste authorities. Tip — most QLD plumbing work uses Form 1 (permit), Form 4 (notifiable, lodged with QBCC) or Form 9 (backflow annual test). Council-specific portals handle online lodgement and fees.

Form 1 — Permit work application (plumbing, drainage and on-site sewerage)
Permit application for work that needs council assessment before it starts (e.g. new connections, upgrades, on-site sewerage). Lodge with the local council that services the property.
Form 2 — Application to amend a permit (incl. extension of time)
Use to amend an existing plumbing permit or request an extension of time before the permit expires.
Form 3 — Covered work declaration
Declaration that covered work (work that will be concealed) was inspected before being covered. Required where the council issued a permit.
Form 4 — Notifiable work
Lodge after completing notifiable plumbing/drainage work (e.g. hot water replacement, fixture replacement). Submit to QBCC within 10 business days; copy goes to the property owner and council.
Form 5 — Testing or commissioning report
Records the results of pressure / commissioning testing carried out on permit work prior to being placed in service.
Form 6 — Remote area compliance notice
Compliance notice for plumbing work carried out in a designated remote area where on-site inspection is impractical.
Form 7 — Notification of responsible person
The licensed plumber/drainer who will be the responsible person for the work nominates themselves to council. Required for most permit work and many notifiable jobs.
Form 8 — On-site sewage work declaration
Declaration that on-site sewage facility work has been completed in accordance with the approval and applicable codes.
Form 9 — Backflow prevention test report
Mandatory annual test report for testable backflow prevention devices. Lodge with the local council. (TradeDesk has a built-in Form 9 module that auto-fills and emails this for you.)
Form 10 — Application for treatment plant approval
Application to the Department of Housing & Public Works for approval of an on-site sewage treatment plant model.
Form 11 — Service report (treatment plant)
Service report for an on-site sewage treatment plant. Submit to council after each scheduled service.
Form 12 — Compliance statement for specialised work
Compliance statement for specialised work (e.g. fire-rated penetrations, medical gas, certain rainwater systems). Signed by the licensed contractor.
Form 14 — Compliance declaration
Final compliance declaration confirming the plumbing/drainage work meets the Plumbing & Drainage Act 2018 and all referenced standards.
Form 17 — Permit or amended permit (issued by council)
The actual permit document issued by council back to the applicant. Useful as a reference template.
Form 18A — Inspection certificate
Inspection certificate issued by the council inspector after a successful stage inspection.
Form 18B — Inspection certificate (fit for use)
Issued where the council certifies the work is fit for use prior to the final inspection.
Form 19 — Final inspection certificate
Council's final inspection certificate confirming the permit work has passed.
Brisbane uses the QLD state forms above and accepts plumbing applications via its online lodgement portal. Trade waste is administered by Urban Utilities.
Gold Coast accepts QLD state forms and runs its own water & trade waste service in-house (Gold Coast Water).
Sunshine Coast Council accepts QLD state forms via its development services portal. Water & trade waste is run by Unitywater.
City of Moreton Bay accepts QLD state forms via the Building & Plumbing portal. Trade waste is administered by Unitywater.
Logan accepts QLD state forms via online lodgement. Logan also runs water & trade waste in-house (Logan Water).
Ipswich accepts QLD state forms. Trade waste is administered by Urban Utilities.
Redland City Council accepts QLD state forms. Water & trade waste is run in-house (Redland Water).
Toowoomba Regional Council accepts QLD state forms via its plumbing development portal.
Noosa Council accepts QLD state forms. Trade waste is administered by Unitywater.
Scenic Rim Regional Council accepts QLD state forms. Trade waste is administered by Urban Utilities.
